CERTIFICATE OF MEDICAL NECESSITY QUESTIONS
| NOTE: Questions in this section should be answered accurately to reflect the true conditions of the patient. Your answers to specific questions are used by Medicare to establish whether specific items are reimbursable. For example, a "YES" answer to Question 1 or 2 and "YES" to 3, 4 & 5 qualifies the patient for Medicare reimbursement.
Question 1: Patients who have severe arthritis of the hip or knee qualify for Medicare reimbursement.
Question 2: Patients who have a severe neuromuscular disease qualify for Medicare reimbursement.
Question 3: Patients who are completely incapable of standing up from a regular armchair or any chair in their home qualify for Medicare reimbursement.
Question 4: Patients who have the ability to ambulate once standing qualify for Medicare reimbursement.
Question 5: All appropriate therapeutic modalities to enable the patient to transfer from a chair to a standing position (e.g. medication, physical therapy) must have been tried and failed, to qualify for Medicare reimbursement.
